History Forms

Things to do prior to your first visit at Family Eyecare Solutions:

1. Complete the age-appropriate history form.
2. Bring the history form to your appointment.
3. Bring a copy of OT (occupational therapist) or educational summaries that you feel may be helpful to your child’s vision assessment.
